Temporary License-Guide to Icons

	Excel or spreadsheet file. Download, open in spreadsheet program, fill in, print it out, and mail in.

Temporary License

The forms below are for use by an organization that does not hold an annual conductors license and wants to apply for a Temporary License to Conduct Charitable Bingo:

			Instructions for Section 1
			Section 1 (Has conducted a temporary game within the past 3 years)
			Instructions for Section 2
			Section 2 (Has never conducted a temporary game or has been longer than 3 years since last temporary license)
			Schedule A (Use to document charitable activities)
			Schedule E Authorization of Representation
			Schedule G (Verification by Parent Organization)
			Schedule I (Volunteer Fire Department affidavit)
			Schedule W (Bingo Prize information certificate)
			Schedule X (Used to provide information for express delivery of license)
			Texas Bingo Quarterly Report (Form TBQR) - (Use this form and this Schedule A to file the required quarterly report for your temporary bingo game(s)).
