Quarterly Reports

A Bingo Chairperson with an active Bingo Service Portal bsp_logo_dark_sm account can file their Conductor Renewal Application online. Conductor Renewal will appear in Bingo Service Portal bsp_logo_dark_sm 45 days from the expiration date. The Charitable Bingo Operations Division (CBOD) will continue to mail Conductor Renewal Applications 30 days from the expiration date.

Amending Your License and Changes to Your Organization

Submit FORMID 11 Conductor License Amendment with all required information and proper signatures.

Conductor License Amendments

An annual or bi-annual license must be amended any time there is a change to the:

  • Bingo Chairperson
  • Organization Name
  • Primary Business Office Location
  • Days and/or Times
  • Playing Location
  • Organization Information

The amendment must be submitted to the Bingo Division prior to the effective change date.

FORMID 11 Conductor License Amendment

Bingo Chairperson

  • Must be an officer or director of the organization.
  • Must be in compliance with the bingo training requirement.
  • Must be current on the worker registry.
  • May not be a designated member of the organization.

Organization Name Change

  • Must submit all amended organizing instruments reflecting the name change.
  • Must submit a copy of the meeting minutes recording that the organization voted to change its name.
  • Must submit a copy of the correspondence sent to the IRS for recording the change on the 501(c) records.
  • IRS letter indicating changes in the organization’s 501(c) exemption status or affiliation.
  • Approval letter from parent organization, if applicable.

Primary Business Office

  • Provide copy of meeting minutes recording voted to move its primary business office to the proposed location name and address and the reason for the move. Meeting Minutes must be signed by an officer.
  • If no business office, the residential address of your chief executive officer.
  • Letter from the parent organization approving primary business office relocation to another county, if applicable.

Primary Business Office Moving to a Non-Adjacent County

  • Provide minutes verified relocation of primary business office to a different county is not solely for relocating its bingo playing location.
  • If moving to a non-adjacent county, at least 20% of the members’ residences must be in the non-adjacent county.

If the playing location is not currently licensed, your amendment will not be granted until the commercial lessor license is approved for the location, if other than a location your organization owns.

FORMID 11 Conductor License Amendment

Days/Times

  • Submit the Conductor License Amendment to change playing times.
  • Ensure playing days and times specified on the form do not conflict with the schedule of any other license at the location or exceed the number of occasions allowed per day or week.
  • Enter the entire playing schedule, including the days and times that are not changing.

Playing Location

  • Provide copy of meeting minutes voting to move playing location and indicating the playing location name and playing location address. Must be signed by an officer.

Note: If the playing location is not currently licensed, your amendment will not be granted until the commercial lessor license is approved for the location, if other than a location your organization owns.

Changes to Your Organization

FORMID 2 Add Individual to a License to Conduct Bingo

  • Add Individuals to a License to Conduct bingo form for newly elected or designated individuals and identify their position held within the organization.
  • FORMID 62 Add Designated Member to Conductor License
  • This form must be submitted by a licensed authorized organization or applicant for a license to conduct bingo desiring to designate an individual as a member of their organization for the purposes of conducting bingo.

FORMID 8 Notice to Remove Individual or Individual Positions

FORMID 60 Change of Mailing Address Notification

This form must be submitted by a licensed organization to change the mailing address, where they will receive all bingo related correspondence form the Charitable Bingo Operations Division (CBOD). Please keep in mind that any change to your operation should be reported to the Commission. Do not wait for renewal time to make changes.

Apply for Temporaries

Regular Annual license holders can apply for temporary occasions where your organization conducts annually or at a different location on the Bingo Service Portal or by submitting FORM ID 16 Licensed Conductor Application for Temporary Bingo Occasions .

Organizations who have held a non-regular Temporary License within the last three years may apply to hold additional occasions on the Bingo Service Portal. The application is available under the 'Organization Information Tab' and selecting 'Application for Temporary Bingo'. Alternatively an organization can submit FORMID 19 Application for Temporary Bingo Occasions for Non-Licensed Organization to the CBOD.

Note: These applications must be received at least 30 working days prior to the first bingo occasion being requested.
