Fingerprint Required  

Individuals holding position(s) marked with an asterisk (*) on FORMID 40 Individual Statement of Manufacturer or Distributor Certification  require the submission of an FBI or DPS fingerprint card when being disclosed for the first time. 

You must comply with the Texas Bingo fingerprint requirements. A fingerprint is required.  Schedule a fingerprint appointment by visiting IndentoGo and your fingerprints will be submitted to the Texas Department of Public Safety and the Federal Bureau of Investigation.  Once completed provide a copy of IdentoGo receipt.    

 

Frequently Asked Questions 

What is a Manufacturer license?  

Manufacturer means: a person who assembles from raw materials or subparts a completed piece of bingo equipment or supplies for use in bingo games in this state; or a person who converts, modifies, adds to, or removes parts from any bingo equipment, item, or assembly to further its promotion or sale for or use in a bingo game in this state. 

Manufacturer’s license required? 

A manufacturer may not sell or supply to a person in this state or for use in this state bingo cards, boards, sheets, pads, or other supplies, or equipment designed to be used in playing bingo, or engage in any intrastate activity involving those items, unless the manufacturer holds a manufacturer's license.   

Any person (sole owner, partnership, corporation or other group) who intends to manufacture or supply in any manner bingo equipment, devices or supplies for use in bingo games in Texas must be licensed. 

What is a Distributor license?  

Distributor is a person (sole owner, partnership, corporation or other group) who obtains, by purchase or otherwise, bingo equipment or supplies for use in bingo in this state and sells or furnishes the items to another person for use, resale, display, or operation. 

Distributor’s license required? 

A distributor may not sell, distribute, or supply bingo equipment or supplies for use in bingo in this state unless the distributor holds a distributor's license. 

Is a Manufacturer or Distributor required to post a bond? 

Only a Manufacturer is required to post a security bond. An applicant for a manufacturer's license must give the commission a cash bond or a bond for $10,000 issued by a surety company chartered or authorized to do business in the state of Texas. The bond must provide for forfeiture to the state on the manufacturer's failure to comply a commission rule or on suspension or revocation of the manufacturer's license. 

What does “whole subsidiary” mean? 

A wholly owned subsidiary is a company that is completely owned by another company. The company that owns the subsidiary is the parent company or holding company. The parent company will hold all of the subsidiary's common stock. 

What form of supporting document, does a Manufacturer or Distributor need to submit to describe the comprehensive organization structure of the business or other affiliated entities? 

Submit an organizational chart, which includes all parent, subsidiary, and related entities, and their relationships along with the FORMID 38 Manufacturer or Distributor Organizational Structure

Does a Salesman for a Manufacturer or Distributor need to submit an FORMID 40 Individual Statement of Manufacturer or Distributor Certification
